Medicare Annual Wellness Visit Nurse (RN/LPN)
West Wichita Family Physicians, P.A. (WWFP)
Location: Wichita, KS 67212
West Wichita Family Physicians is seeking a compassionate, organized, and self-motivated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our Team-Based Care Department as a Medicare Annual Wellness Visit Nurse.
This unique position focuses on improving the health and well-being of our aging patient population through comprehensive Medicare Annual Wellness Visits. The successful candidate will play a key role in preventive care, chronic disease management, patient education, and care coordination while working closely with physicians and the healthcare team.
Annual Wellness Visits are conducted in the clinic or virtually, providing a comprehensive assessment of each patient's overall health status, preventive care needs, cognitive health, safety risks, and future healthcare planning. Information gathered during these visits is communicated to the patient's primary care physician and used to develop personalized care plans.
This position is responsible for helping achieve patient care and quality goals associated with Medicare Annual Wellness Visits and is an integral part of our primary care team.
Essential Responsibilities
  • Conduct Medicare Annual Wellness Visits in-office or virtually.
  • Complete comprehensive health assessments, including preventive care reviews, cognitive screenings, fall risk assessments, and environmental safety evaluations.
  • Identify gaps in preventive care and recommend appropriate screenings, vaccinations, and follow-up services.
  • Educate patients regarding preventive health measures, chronic disease management, and healthy lifestyle choices.
  • Collaborate with physicians and care team members to communicate assessment findings and recommendations.
  • Coordinate follow-up care and referrals as needed based on patient needs identified during wellness visits.
  • Maintain accurate and timely documentation within the electronic medical record.
  • Track and manage assigned patient populations to ensure completion of annual wellness visits and quality measures.
  • Assist in achieving departmental quality and performance goals.
  • Maintain patient confidentiality and adhere to all clinic policies and regulatory requirements.
Qualifications
Required
  • Current Kansas RN or LPN license in good standing.
  • Strong computer and electronic medical record skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently while maintaining strong collaboration with physicians and staff.
Preferred
  • Experience in primary care, family medicine, geriatrics, care management, population health, or preventive care.
  • Experience conducting patient assessments and care coordination activities.
  • Knowledge of Medicare Annual Wellness Visit requirements and quality measures.
