Job Summary: With responsibility of the care of our Participants 24 Hours, 7 days a week, this position work on-call and will be responsible for the telephonic assessment and in person assessment as needed.


Essential Functions:

  • Responsible for on-call phone calls and communication that occurs involving Participant care through our on-call system.
  • Be able to utilize clinical knowledge and critical thinking to determine best course of action when receiving on-call phone calls.
  • Ensure high quality care and communication is delivered at all times.
  • Provide direct care to the patients, including triage, administering treatments and medications.
  • Assist in development, follow and coordinate each Participant comprehensive assessment and plan of care.
  • Notify physician, when necessary, of changes in the patients condition, weight, medication errors, medication management issues, abnormal laboratory/x-ray results and any other unusual observation.
  • Complete all necessary documentation according to regulations and LIFE Pittsburgh’s policies/procedures.


Working Conditions:

  • Works within the LIFE Pittsburgh Adult Day Health Centers (ADHC), or occasional travel between centers for other related functions.


Requirements:

Knowledge /Skills / Abilities

  • Ability to obtain and interpret information in terms of the specific needs of the Participant.
  • Frequently required to manage many details within a fast paced environment
  • Possesses knowledge of the Human Anatomy and development of the Participant served.
  • Understands the range of treatment needed by the participant served.
  • Well-developed organizational and interpersonal skills. Strong written and verbal communication skills are needed.
  • Ability to multi task and problem solve complex situations
  • Must be able to support cognitively impaired seniors and their families to deal with sensitive issues and facilitate problem solving.
  • Must be flexible and able to change easily.
  • Ability to work with people from various social- cultural backgrounds effectively


Required Certification/License:

  • Valid PA Registered Nurse License
  • Valid CPR certification
  • Valid Driver’s license and automobile insurance
  • Must have a vehicle for travel


Education/Experience:

  • Graduate of an accredited school of nursing (Bachelors preferred)
  • Minimum (1) year experience working with the frail or elderly
  • Experience working with and relating to the geriatric population and their family members
