Position Summary

The Weekend On-Call RN is responsible for providing timely triage, support, and skilled nursing visits to hospice patients and families after normal business hours. This role ensures continuity of care, symptom management, and compassionate support throughout the weekend on-call period.

Essential Job Responsibilities

- Provide on-call hospice nursing coverage from Friday at 4:30 PM until Monday at 8:30 AM.

- Respond promptly to triage calls, patient/family needs, and emergent situations.

- Make in-person skilled nursing visits for symptom management, patient decline, death visits, or urgent concerns.

- Assess, implement, and evaluate patient care plans in collaboration with IDT.

- Deliver education and emotional support to patients, caregivers, and families.

- Complete documentation in the EMR accurately and promptly.

- Communicate changes in patient condition to the Medical Director, and IDT.

- Assist with admissions, pronouncements, and transitions of care when required.

- Ensure timely handoff report on Monday morning.

Qualifications

- Current RN license in the state of Oklahoma.

- One year of hospice, palliative care, or home health experience preferred.

- Strong assessment and critical-thinking skills.

- Ability to work independently in the field.

- Valid driver’s license, reliable transportation, and auto insurance.

- Compassionate communication skills.

- Weekend on-call role with on-call, daily, admission visits possible

- Must work the full on-call window Friday 4:30 PM to Monday 8:30 AM.

- Must respond promptly to patient needs during on-call hours.

- Travel required within TruHeart’s service area.
