Job description

Schedule: 10 hour shifts 8am to 6pm, Friday–Sunday or Saturday–Monday

Administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care in conjunction with the Registered Nurse Case Manager and under the direction and supervision of Clinical Team Manager. The registered nurse executes the plan of care as established by the Registered Nurse Case Manager and makes necessary revisions as needed, furnishes those services requiring substantial and specialized nursing skill, provides appropriate preventive and rehabilitative nursing procedures, prepares clinical and progress notes, informs the physician and other personnel of changes in the patient's condition and needs, counsels the patient and family in meeting nursing and related needs, participates in in-service programs, and supervises and teaches other nursing personnel.

RESPONSIBILITIES:

  • Assesses and charts observations of the patient’s condition at each visit.
  • Completes evaluation tasks, including reviewing medication, vital signs, and complete head to toe assessment.
  • Administers skilled nursing treatments and interventions including but not limited to at-home IV infusion, wound assessment, wound care, and education aimed at promoting and maintaining patient independence.
  • Revises plan of treatments and discharge plan in conjunction with registered nurse case manager, patient, family, physician, care delivery team, and third party payors including obtaining medical orders and approval of medical services.
  • Consults with and educates the patient, family and other team members regarding disease process, self-care techniques and preventive strategies.
  • Assures the completion of all appropriate clinical records needed for compliance with state and federal legislation and the delivery of seamless service within established timeframes.
  • Demonstrates flexibility and willingness to visit patients based on need and not geography.
  • Ensures problem resolution at the point of contact.
  • Monitors results, progress, and takes corrective action on quality indicators, including the level of customer satisfaction with the RN function.
  • Responds to findings of various Quality Management reviews/audits and ensures resolution of potential problem areas.
  • Participates in peer review and Quality Assessment and Performance Improvement as assigned.
  • Maintains productivity requirements.
  • Ensures that appropriate visit authorizations are in place and that visit documentation reflects the medical necessity for skilled nursing care to be provided in the home setting.
  • Participates in education including orientation, in-service training programs and self- development, as needed.
  • Responsible for the Key Performance Indicators (KPI’s) established by their manager.
  • Performs other duties as requested.

QUALIFICATIONS:

  1. New York State Licensed RN.
  1. One year of experience as a clinical nurse in a hospital or other direct patient care clinical setting required.
  1. Strong critical thinking skills, the ability to follow standards/procedures, and make appropriate clinical judgments to provide safe and effective client care.
  1. Ability to work independently with minimal supervision, adapt to change, and manage a variety of assignments while establishing priorities and ensuring the necessary duties are completed.
  1. Strong interviewing, clinical assessment and communication skills.
  1. Ability to exercise initiative and independent judgment.
  1. Ability to build and maintain professional relationships
  1. Current NYS driver’s license

Physical Requirements: The position involves frequent sitting, standing, and walking. Occasional bending and pushing may be required. Tasks such as kneeling, squatting, reaching, twisting, balancing, climbing, and pulling are required infrequently.

Salary: $70,824 to $83,220
