Job Description:

This position is responsible for operational management of Intermountain @ Home Hospital Level Care's strategy and care delivery. Leads effective coordination with system strategies, resources, IT, Clinical Programs, and regional teams. Assists with the HLC at Home strategic planning, business development, and growth opportunities for Intermountain Healthcare, specifically in relation to Hospital Level Care at Home to enhance access, improve clinical outcomes, reduce the cost of care and gain efficiency that adds value to the healthcare proposition. This position leads the Hospital Level Care at Home efforts to strengthen and enhance relationships between facilities and providers. This requires frequent interaction with administrative leaders, clinical leaders, and subject matter experts throughout Intermountain and Select Health. Facilitates health care services that provide exceptional quality while maintaining a cost structure that is beneficial to Intermountain, its patients, and community. Ensures that policies, procedures, activities, and strategies are consistent with and coordinated with those of Intermountain Healthcare.*

This system-wide role directs or works with system-wide steering committees, hospitals, homecare, virtual hospital, telehealth, departments, councils, and/or teams to operationalize projects related to improving the clinical process, quality outcomes, and value to the patient and patient experience through telehealth services. The position leads the implementation of Hospital Level Care at Home projects and initiatives throughout the system including hospitals, clinics, health plans, and home care.

We are seeking candidates with experience in a hospital or healthcare setting, such as nursing, social work, patient care, healthcare operations, or other clinical support roles. The ideal candidate will have an understanding of the inpatient care environment and the ability to collaborate effectively with clinical teams.

Work Schedule

  • Hybrid role - Position requires on-site presence at times for in-person rounding and meetings, with flexible scheduling.
  • Core business hours: Monday-Friday, 8:00 a.m. - 5:00 p.m.


Job Essentials

  • Collaborates with HLC at Home Operational Leadership and frontline caregivers, to plan, develop, and implement new services.
  • Manages work of Project Coordinator in developing business plans for new services. Leads and supervises department or program staff. Responsible for staffing, coaching, development, performance management, and administrative duties required of those in a supervisor role.
  • Develops metrics, dashboards, and monthly reports on performance of all services in portfolio. Develops service and engagement standards and metrics for each service. Identifies issues and improvement opportunities in support of Board/system strategies, and develops solutions including tactics; costs; measures and outcomes. Leads or collaborates with others to define IS development and project management components.
  • Develops and implements education, communication and/or implementation plans associated with assigned telehealth initiatives. Collaborates with human resources, finance, and management engineering in developing and implementing activities related to projects. May facilitate appropriate external and internal communications related to projects (e.g., press queries, Intermountain Stories regular column, promotional strategies, etc.).
  • Coordinates internal resources to identify and operationalize internal and external related best practice initiatives. Coordinates necessary resources to resolve complex problems and balances the focus on processes with appropriate outcomes. Collaborates with clinical program leaders and staff to ensure we both lead and support all clinical program initiatives.
  • For projects requiring information technology/software solutions, coordinates with IT staff and clinical users, and provides ongoing evaluation to assure success. Recognizes barriers to project plans and determines mitigation strategies. Manages supplier relationships and product performance.
  • Establishes or develops appropriate metrics for initiatives. Serves as a content expert and resource to all levels to provide support, education and clarity. Evaluates and quantifies the impact of implemented projects through available measurements relative to clinical process, outcomes, patient or staff experience, and/or financial objectives.
  • Accountable for financial performance, budgeting, and reporting for services within portfolio. Oversees services and resources related to projects including RFP process and contracting reviews. Develops policies, guidelines, and documentation relative to project initiatives.
  • May be responsible for the Patient Safety Improvement Program. Participates in the development and maintenance of the Intermountain Healthcare Quality and Patient Safety Plan.
  • Acts as content expert in Continuous Improvement methodologies and facilitates and educates to the utilization of these tools and methodologies. Provides input and consultative services in the development, implementation, and monitoring of patient safety improvement projects for the system and/or region and individual business units. Creates tools and resources to ensure standardization of improvement processes and monitoring.

Minimum Qualifications

  • Bachelor's degree. Education must be obtained through an accredited institution. Degree will be verified.
  • Experience managing a clinical team
  • Willingness and the ability to travel throughout the system to meet with facility leadership to accomplish work as necessary.


Preferred Qualifications

  • Formal training or education in process improvement, change management, project management, or a related field.
  • Professional healthcare or experience in a clinical setting that demonstrates clinical expertise and understanding of healthcare systems.
