Job description

Job Summary: Provides transportation to and from healthcare facilities for residents of Skilled Nursing Facility. This Job also will require some maintenance to the facility at times.

Duties: Works with management to provide transportation to residents based on transportation schedule.

Provides, completes, and documents resident care as assigned in a timely and accurate manner.

Attends and participates in facility in-service programs about transportation needs. 

Reports any change of condition to charge nurse.


  • Must be capable of independently working on the following for transportation

  • Transporting residents to and from appointments

  • Moving residents about the facility

  • Moving residents in an out of hospitals, and healthcare facilities

  • Managing transportation schedule

  • Communicating key information to facility team regarding transportation

Transportation position requires a valid drivers license

Employee must be able to push, pull, move residents to and from wheel chair.
