Job Description Job Title: RN Case Manager - Home Health Location: Culver City, CA (Coverage Area: Beverly Hills, Hollywood, Inglewood) Employment Type: Full-Time | Onsite Schedule: Monday-Friday + On-Call (1 weekend per month) Compensation
  • Pay Per Point Model: $114,340.66 - $128,652.29 annually (based on productivity)
  • Minimum Productivity: 28 visits per week
  • Typical Range: 28-40 visits per week
Position Overview Wheeler Staffing Partners is seeking a Registered Nurse (RN) Case Manager for a home health opportunity in Culver City, CA. This role is responsible for delivering high-quality, patient-centered care in the home setting while managing a caseload and coordinating care across an interdisciplinary team. The ideal candidate is compassionate, organized, and skilled in clinical assessment, care planning, and patient education. This position offers the opportunity to provide meaningful, one-on-one care while supporting patients in maintaining independence and improving quality of life. Key Responsibilities
  • Provide skilled nursing care to patients in their homes in accordance with physician orders
  • Conduct comprehensive patient assessments and develop individualized care plans
  • Coordinate care with physicians, therapists, and other healthcare professionals
  • Monitor patient progress and adjust care plans as needed
  • Educate patients and families on disease management, medications, and treatment plans
  • Ensure accurate and timely documentation in compliance with regulatory standards
  • Promote patient independence and improve overall quality of life
  • Manage assigned caseload and maintain productivity expectations
Qualifications
  • Graduate of an accredited school of professional nursing
  • Active Registered Nurse (RN) license in the state of practice
  • Minimum of 1 year of RN experience required
  • Home health experience preferred but not required
Required Certifications and Licensure
  • Current Registered Nurse (RN) license in the state of California
  • Current CPR certification
  • Valid driver's license, reliable transportation, and active auto insurance
  • Ability to travel to patient homes within assigned coverage area
