We strive to hire excellent people at Concordia - and keep them as well!
Spiritual Care Coordinator: Home Health and Hospice
Concordia Hospice of Washington & Concordia Visiting Nurses
At Concordia, faith is at the heart of everything we do. We believe spiritual care is an essential part of healing and hospice-bringing peace, hope, comfort, and compassionate presence to patients and families during some of life's most meaningful moments.
We are seeking a part-time Spiritual Care Coordinator who feels called to serve others through faith, compassion, and whole-person care. Whether supporting patients receiving hospice services or home health care, you will provide spiritual guidance, emotional support, and a comforting presence while honoring each individual's beliefs, values, and traditions.
Why Choose Concordia?

  • Supportive interdisciplinary team including nurses, social workers, therapists, aides, and volunteers
  • Flexible part-time schedule
  • Independence in the field while remaining connected to a caring and collaborative team
  • The opportunity to use your gifts to bring hope, comfort, and peace during life's most vulnerable moments
What You'll Do
  • Provide spiritual care, prayer, counseling, and emotional support to patients, families, and caregivers
  • Assess spiritual needs and incorporate them into each patient's plan of care
  • Offer comfort through presence, conversation, scripture, prayer, or other meaningful spiritual practices while respecting all faith traditions and belief systems
  • Support patients and families through illness, grief, end-of-life care, and times of transition
  • Collaborate with nurses, social workers, aides, therapists, physicians, and other members of the interdisciplinary care team
  • Participate in care planning meetings and document spiritual care visits
  • Lead or assist with memorial services, bedside prayers, and family support as appropriate
Who You Are
You lead with compassion, empathy, and grace. You understand that every patient's spiritual journey is unique and are committed to providing individualized support that honors their beliefs while reflecting Concordia's Christian mission.
We are seeking someone who has:
  • Ordination, ecclesiastical endorsement, chaplaincy experience, pastoral care experience, or a background in ministry or spiritual care
  • Experience in hospice, home health, healthcare, church ministry, or a related setting preferred
  • A compassionate, patient-centered approach with excellent listening and communication skills
  • The ability to minister to and support individuals from diverse faith traditions and backgrounds
  • Strong documentation and organizational skills
  • The ability to work independently while collaborating with an interdisciplinary team
  • A valid driver's license and reliable transportation for travel throughout the service area
