Bright Path Social Work is seeking a dedicated Licensed Master Social Worker (LMSW) for a full-time remote position in Toms River, New Jersey. This position allows for a flexible remote work environment tailored to meet the specific needs of our clients and their families. While the primary work setting is virtual, collaboration with school personnel and regular check-ins with clients may involve occasional onsite meetings or visits to ensure comprehensive care. A full-time schedule is expected, with standard hours from Monday to Friday, 8:00 AM to 4:30 PM. Flexibility in scheduling will be necessary to accommodate client visits, and on-call duties may be required to address urgent situations.

• Conduct in-depth assessments of seniors' psychosocial needs, focusing on emotional, mental, and social aspects of their lives.
• Collaborate with seniors and their families to formulate personalized care plans that align with their goals and preferences, considering their unique circumstances.
• Deliver empathetic counseling and emotional support to help clients navigate significant life transitions, such as aging, health challenges, or loss.
• Advocate for clients' rights, ensuring they have access to vital community resources and support services.
• Engage with interdisciplinary teams, including educators and healthcare professionals, to enhance the support provided to senior students.
• Regularly monitor clients' well-being through visits or virtual check-ins to identify any concerns in their living environments and address safety issues.
• Facilitate effective communication and resolve any conflicts within family dynamics to improve caregivers’ involvement.
• Educate clients and their families about available resources, community support groups, and programs that can foster their social engagement.
• Maintain meticulous documentation of assessments, care plans, and client interactions in compliance with legal and ethical standards.
• Stay informed of best practices in social work, including policies affecting aging individuals, ensuring adherence to ethical principles in all interactions.

The primary clientele for this position includes seniors residing in our community who may face various challenges related to aging, mental health, and social isolation. By bridging the gap between individual needs and available resources, the Social Worker will work to ensure that every senior client has the opportunity to lead a fulfilling and vibrant life.

• Master’s degree in Social Work from an accredited program.
• Current and valid LMSW certification recognized by the state of New Jersey.
• A minimum of two years of relevant experience in social work, particularly in community settings or geriatric care.
• Solid understanding of social work principles, particularly as they pertain to aging and mental health issues.
• Outstanding communication, counseling, and problem-solving skills.
• Empathy and relational skills to foster trust with clients and their families.
• Effective collaboration abilities with interdisciplinary teams and community agencies.
• Proficiency in electronic health record (EHR) systems and other digital tools necessary for remote service delivery.

• Experience working within a school or educational environment is highly favored.
• Knowledge of resources specific to seniors available in New Jersey and experience with community outreach initiatives.
• Skills in conflict resolution and facilitation of family meetings related to client care.
