Bright Path Social Work is seeking a dedicated and compassionate Licensed Master Social Worker (LMSW) to join our team in providing crucial support for seniors within the Orlando community. Work Environment

This remote position allows for flexibility while being part of a dedicated team focused on delivering exceptional social work services. You will be responsible for conducting assessments and service coordination as part of an integrated care approach, all from the comfort of your chosen workspace.

Key Responsibilities

• Conduct comprehensive assessments of seniors’ psychosocial needs, integrating emotional, mental, and social health considerations.
• Develop and implement personalized care plans in collaboration with clients and their families, ensuring alignment with individual goals.
• Provide individual counseling and emotional support, helping seniors manage transitions and navigate challenges such as health changes, loss, and family dynamics.
• Advocate for clients' rights and ensure access to community resources, including healthcare, legal aid, and support groups.
• Work closely with interdisciplinary teams to coordinate services, optimizing the care provided to our clients.
• Regularly monitor clients’ well-being through virtual check-ins, identifying emerging issues or deteriorations in condition promptly.
• Facilitate effective communication among family members involved in a senior client’s care to mitigate conflicts and enhance support systems.
• Educate clients and families on available community resources to promote social engagement and enhance the quality of life.
• Maintain meticulous records of client assessments, care plans, progress notes, and any relevant interactions in line with ethical standards.
• Stay abreast of current practices and regulations in social work to ensure compliance and high-quality care delivery.

This role primarily focuses on seniors, addressing their diverse and complex needs. You will provide services that encompass physical health, emotional well-being, and social connectivity, striving to improve their overall quality of life. Qualifications

• A Master’s degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Master Social Worker (LMSW).
• A minimum of two years of practical social work experience, preferably in a community health setting or geriatric care.
• In-depth knowledge of social work principles, aging-related issues, and community resources available for seniors.
• Strong counseling, empathetic communication, and problem-solving skills.
• The ability to build rapport and demonstrate compassion towards seniors and their families.
• Exceptional teamwork and collaboration abilities within interdisciplinary health care teams.
• Proficiency in electronic health record (EHR) systems and other relevant technological tools.

Preferred Experience

• Previous experience in a community health clinic or a geriatric-focused environment.
• Familiarity with local community resources and programs specifically geared towards senior populations.
• Experience working with diverse populations and cultural sensitivity in addressing their needs.
• Background in crisis intervention and conflict resolution skills.

Compensation and Benefits

Bright Path Social Work offers a competitive salary range of $60,000 to $100,000 per year, commensurate with experience.
