RN Case Manager | Apple Valley, CA

Location: Apple Valley, California
Assignment Length: 13 Weeks
Schedule: Day Shift | 5x8 Hours | 8:00 AM - 4:30 PM

Job Summary

"Navitas Healthcare, LLC" is seeking an experienced RN Case Manager for an acute care hospital assignment in Apple Valley, CA. The ideal candidate must have strong hospital-based case management and discharge planning experience with the ability to work independently in a fast-paced healthcare environment.

Required Qualifications
Active California RN License required
Minimum 2 years of Acute Care Case Management / Discharge Planning experience required
Hospital experience required
First-time travelers will not be considered
Strong communication and coordination skills
Ability to manage high patient caseloads effectively

Key Responsibilities
Coordinate patient discharge planning and continuity of care
Collaborate with physicians, nursing staff, and interdisciplinary teams
Review patient care plans and utilization needs
Arrange post-discharge services and resources
Maintain accurate patient documentation and case records
Support patients and families throughout hospitalization and discharge process
