Silence Aloud Inc/ The Bridge Wellness Center is looking for a dedicated and compassionate Case Manager to join our growing team in Moreno Valley, CA. This is an exciting opportunity to make a meaningful impact in the lives of individuals by connecting them with the resources and support they need. Monday- Friday 10am-6:30pm

Responsibilities:
  • Assess client needs and develop individualized care plans to support their goals and well-being
  • Coordinate and monitor delivery of services across multiple community and social service agencies
  • Maintain accurate and up-to-date client records, case notes, and documentation
  • Serve as a primary point of contact and advocate for clients throughout the case management process
  • Conduct regular follow-ups and progress evaluations to ensure clients are meeting milestones
  • Collaborate with internal teams, external providers, and community partners to ensure holistic client support
  • Identify and connect clients with appropriate community resources, programs, and referrals
  • Conducts assessments and develops service planning

Requirements:
  • Bachelor's degree in Social Work, Psychology, Human Services, or a related field preferred
  • Prior experience in case management, social services, or a related role
  • Strong knowledge of community resources and social service systems in the Moreno Valley/Inland Empire area
