Registered Nurse Case Manager (RNCM)

Join a compassionate and supportive hospice team dedicated to making a meaningful difference in the lives of patients and families. The Care Team Hospice is seeking a full-time Registered Nurse Case Manager to provide skilled nursing care and coordinated end-of-life support to patients in their homes and facility settings while collaborating closely with the interdisciplinary team.
Schedule, Hours & Territory
โ€ข Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
โ€ข Crown Point, IN, and the surrounding communities within approximately a one-hour driving radius.
โ€ข RN Case Managers may participate in an on-call rotation with additional pay
โ€ข Some branches offer dedicated on-call staff, resulting in backup on-call coverage only

Key Responsibilities
โ€ข Conduct comprehensive patient assessments and develop individualized plans of care
โ€ข Provide skilled nursing care in accordance with hospice standards and physician orders
โ€ข Coordinate care with physicians, caregivers, and interdisciplinary team members
โ€ข Educate patients and families on symptom management, disease progression, and end-of-life care
โ€ข Document assessments, interventions, and care plans accurately and in a timely manner in the EMR HCHB (Homecare Homebase)
โ€ข Participate in interdisciplinary team meetings and care planning
โ€ข Support after-hours patient needs as part of an on-call rotation when applicable
โ€ข Active Registered Nurse (RN) license
โ€ข Hospice or home health experience preferred
โ€ข Strong clinical assessment and care coordination skills
โ€ข Ability to work independently in the field
โ€ข Strong communication and documentation skills
โ€ข Experience using HCHB (Homecare Homebase) preferred
Field Role Requirements
โ€ข Active driver's license
โ€ข Ability to pass a criminal background check
โ€ข Ability to pass a Motor Vehicle Record (MVR) check
โ€ข Current TB testing in accordance with agency policy
โ€ข Current CPR certification as required for the role
