Key Responsibilities
  • Supervise and support Clinical Coordinators to ensure optimal patient care

  • Oversee individualized patient care plans and ensure timely updates

  • Address staff and patient inquiries, offering clinical guidance

  • Triage patient calls and prioritize clinical concerns

  • Implement and enforce home health clinical policies and procedures

  • Ensure accurate, timely documentation and compliance with regulations

Qualifications
  • Active California RN License (Required)

  • Home Health experience (Required)

  • 1-2 years of supervisory experience (Preferred)

  • BLS Certification (Preferred)

  • Excellent organizational, communication, and interpersonal skills

  • Strong computer proficiency (Apple products & EMR systems)

  • Ability to multitask and manage high call volumes with professionalism


Education
  • Associate Degree in Nursing (Required; BSN Preferred)
