Job description

Job Summary:
Desire Home Care, Inc. is seeking a compassionate and experienced Licensed Vocational Field Nurse (LVN) to join our team as a shift healthcare employee. This individual contributor role will be based in the Riverside, Fontana, and Murrieta areas and will involve providing healthcare services to patients with intellectual and/or developmental disabilities in the comfort of patients' homes. As an LVN Home Health Shift Nurse, you will be responsible for providing treatment, medication administration, and supervision according to their specific health needs, while also adhering to all regulatory and ethical standards.
Compensation- Shift Care:

  • Routine Follow-Up Visit: $25-30 per hour

Responsibilities:

  • Provide skilled nursing care to patients in their homes, in accordance with established care plans and physician's orders
  • Monitor and assess patients' health status and report any changes or concerns to the supervising Registered Nurse
  • Administer medications and treatments as prescribed by the physician
  • Provide wound care, catheter care, G-tube care, and other nursing procedures as needed
  • Educate patients and their families on their conditions, medications, and self-care techniques
  • Collaborate with other healthcare professionals to coordinate and manage patient care
  • Maintain accurate and up-to-date patient records, including progress notes, medication administration records, and nursing assessments
  • Adhere to all regulatory and ethical standards, including HIPAA guidelines and infection control protocols
  • Communicate effectively with patients, families, and other team members to ensure the best possible outcomes for patients

Requirements: *New Graduates Welcomed*

  • Active and unrestricted LVN license in the state of California
  • Minimum of 1 year of experience as an LVN, preferably in a home health setting
  • Have experience working with patients diagnosed with autism, cerebral palsy, down syndrome, and/or epilepsy
  • Have experience working in pediatrics
  • Strong knowledge of nursing principles, practices, and procedures
  • Excellent clinical assessment and critical thinking skills
  • Ability to work independently and manage time effectively
  • Strong communication, interpersonal, and organizational skills
  • Must have a valid driver's license and reliable transportation
  • Must have current BLS certificate
  • Physical and TB clearance valid within the year
  • Ability to complete documentation via agency EMR system

SHIFTS: Minimum of 6 hour shifts, no more than 8 hours, no holiday shifts

Morning, Afternoon, and Evening

To achieve a full 8-hour workday, you may be assigned multiple patients, and shifts typically vary in length
