Job description

GENERAL PURPOSE

Oversees all aspects of the Residential Program including, but not limited to: supervises Program Managers and Nurses to ensure quality service delivery; recruits new Host Home Providers; completes intake of persons in need of residential services; provides orientation training to new residential program staff and independent contractors; ensures compliance with state and local regulations; serves as liaison between CLA and the community.

I.                             Essential Duties and Responsibilities

 

A.     Leadership & Supervision

1.       Hires, trains, supervises, and evaluates Program Managers and Nursing staff.

2.       Provides coaching, performance management, and team development.

3.       Participates in and oversees the on-call rotation, providing leadership during medical, behavioral, and other emergencies.

4.       Supports interdisciplinary collaboration and attends IDT and Service Plan meetings as needed.

B.      Regulatory Compliance & Quality Assurance

1.       Ensures compliance with all applicable state and local regulations governing residential services.

2.       Oversees required home visits, monitoring reports, file reviews, and follow-up from surveys or CCB reviews.

3.       Monitors trends, implements corrective actions, and ensures timely resolution of compliance concerns.

4.       Oversees tracking of medical protocols, informed consents, Health and Safety Plans, Human Rights Committee submissions, and nurse delegation requirements.

 

C.      Medical & Documentation Oversight

1.       Collaborates with Nursing and Program Managers to ensure medical orders, MAR documentation, and after-visit summaries are accurate and implemented.

2.       Reviews and approves incident reports and ensures timely follow-up.

3.       Provides guidance and corrective action related to documentation and provider performance.

 

D.     EMR & Systems Oversight

1.       Serves as system administrator for the agency’s EMR (Therap), overseeing access, training, and documentation integrity.

2.       Ensures required documentation is maintained in accordance with regulatory standards.

 

E.      Fiscal & Administrative Oversight

1.       Reviews and approves attendance records for billing and provider payment.

2.       Approves financial vouchers and selected reimbursement requests.

3.       Assists in oversight of personal needs funds and ABLE account coordination.

4.       Collaborates on contract development, policy updates, and administrative process improvements.

5.       Collaboratively works with the Executive Director and Financial Controller to develop the Residential Program Budget.

 

 

F.      Provider & Service Coordination

1.       Oversees respite coordination processes and provider compliance with training and contractual requirements.

2.       Supports performance improvement strategies for Host Home Providers and Family Caregivers.

 

G.     Community & Agency Development

1.       Collaborates with leadership and marketing staff to support agency growth, partnerships, and community outreach initiatives.

II.                  Recruitment/Intake

A.     Recruits new Host Home Providers

1.       Develops advertisements with Marketing Director.

2.       Develops new relationships with potential Hsost Home and Family Caregiver providers through website, email, phone call, and word of mouth inquiries.

3.       Reviews submitted applications and conducts interviews with potential Host Home Providers and Family Caregivers.

B.      Coordinates Host Home Provider  background screening, the collection of required documents and trainings for completion prior to contract signing.

C.      Develops and facilitates Host Home Provider and Family Caregiver basic training classes to meet regulatory requirements. 

D.     Reviews the completion of Host Home Provider and Family Caregiver contractual files prior to the completion of provider contract signing.

E.      Enters new care providers into the EMR (Therap) system, generates log-in credentials, assign user permissions, provides training on electronic documentation, system communication,  and navigation.

F.      Assists and ensures orientation is completed for new Host Home Providers, Family Caregivers  and agency staff.

G.     Collaborates with Host Home Providers to create profiles to be utilized in the matching process with potential people coming into residential services.

H.     Intakes new PRS

1.          Reviews and responds to potential leads through CCB RFPs as well as website, phone, and word of mouth inquires. Entering active lead information into tracking system.

2.          Meets with the person seeking services, their families and or support teams to determine needs and personal goals to determine if CLA can effectively provide services.

3.          Completes matching process of the person seeking services with potential Host Home Providers.

4.          Ensures the necessary service plan revisions have been made for funding of services and all necessary paperwork is submitted to the CCB prior to placement.

5.          Complete thorough intake process gathering all necessary information pertaining to the continuity of care and support  for Medical, Psychological, Behavioral, ADL, and Financial Benefit needs. This includes completing an explanation and signing of the rights, policies, disclosures, and agreements with the person in services.

6.          Actively provide program management support exclusively to new people in services for the first 30 to 60 days to ensure stabilization before transition to a program manager caseload.

IV.      Attends all required staff meetings, Board of Director meetings, CCB service agency meetings and other meetings and training as requested by the Executive Director

V.        Other duties as required.

REQUIRED QUALIFICATIONS

Bachelor’s degree in Human Services or related field, plus two years of experience working with people who have intellectual/developmental disabilities or a minimum of five years of experience in residential services for individuals with intellectual/developmental disabilities; experience in development and management; ability to lead and work on a team; organized and detail oriented; ability to negotiate and handle conflicts effectively; excellent written and verbal communication skills; flexibility in work schedule; valid Colorado driver’s license, a reliable vehicle and auto insurance; working knowledge of excel and word; knowledge of Therap preferred.

We currently provide residential services to 32 people and day program services to 22 people.
