Who We Are
Imagine Pediatrics is a tech enabled, pediatrician led medical group reimagining care for children with special health care needs. We deliver 24/7 virtual first and in home medical, behavioral, and social care, working alongside families, providers, and health plans to break down barriers to quality care. We do not replace existing care teams; we enhance them, providing an extra layer of support with compassion, creativity, and an unwavering commitment to children with medical complexity.
The primary location for this role is remote with potential for quarterly travel. Expected schedule will be 8:00am-5:00pm Mountain Time (preferred) or Central Time.
What You'll Do
As a supervisor and integrated care team leader, the Supervisor, Medication Support Navigator oversees and coordinates medication-related services to improve outcomes for pediatric patients with chronic medical and behavioral health conditions. This role provides people leadership to the Medication Support Navigator team, including shift scheduling, performance management, and staff development. The Supervisor, Medication Support Navigator works closely with the Clinical Pharmacy Manager to ensure operational excellence and staff competency, facilitate process improvements, and quality oversight. The role maintains hands-on involvement in patient care by collaborating with Clinical Pharmacists, Medication Support Navigators, and cross-functional care teams for care coordination, maintenance of patient medication therapy records, troubleshooting medication access barriers with community pharmacies and pharmacy benefits managers, and outreach to patient caregivers. In this role, you will:
  • Function in the Medication Support Navigator - Care Coordinator role supporting both regions and the Medication Navigator team
  • Coordinate referral tasking to Medication Support Navigator team members
  • Ensure timely completion of referrals and supports Medication Support Navigator team in timely completion
  • Interact with cross-functional teams for patient care
  • Facilitate appointment scheduling for pharmacists
  • Follow referrals and follow-up assigned to team for rebalancing and in support of daily operations
  • Lead AM huddle with Clinical Pharmacy Manager
  • Facilitate operational improvement in Medication Support Navigator processes, in coordination with the Clinical Pharmacy Manager
  • Subject Matter Expertise for Medication Support Navigator operations and processes
  • Maintain Medication Support Navigator shift schedule, time & attendance, PTO management
  • Direct people leadership of the Medication Support Navigator team including 1:1s and performance management
  • Ensure operational excellence for the Medication Support Navigator team through monitoring individual contributor performance metrics and quality checks
  • Onboard new Medication Support Navigators
  • Support Clinical Pharmacy Manager to ensure initial and ongoing competency of Medication Support Navigator team members
  • Perform other duties as assigned

What You Bring and How You Qualify:

In this role, you will need:
  • High School Diploma or equivalent
  • Current accredited pharmacy technician certification (such as PTCB)
    • Maintenance of certification expected
  • Registration with applicable state board(s) of pharmacy as required by law
    • Maintenance of all technician registration for all assigned states expected
  • 5+ years of experience as a certified pharmacy technician
    • Experience in a call center or an ambulatory clinic setting, highly preferred
  • 3+ years of experience in people leadership
    • Leading pharmacy technicians in a call center environment highly preferred
  • Experience and/or team management in a virtual environment, preferred
  • Strong Microsoft suite proficiency
  • Proficiency with electronic health record systems
  • Bilingual in Spanish and English highly preferred

The role offers a base salary range of $65,000 - $80,000 in addition to competitive company benefits package and eligibility to participate in an employee equity purchase program (as applicable).
