• Conduct comprehensive psychosocial assessments that gather information through interviews, observations, and medical records to understand the multifaceted influences on each patient's recovery journey.

• Work closely with the rehabilitation team to formulate and implement individualized treatment plans that address each patient's unique psychosocial needs.
• Guide patients in accessing community resources such as support groups, vocational rehabilitation, and housing assistance, promoting comprehensive post-recovery support.
• Educate patients and their families about available community resources and post-rehabilitation services that can aid in sustaining recovery and enhancing quality of life.
• Assist in discharge planning, ensuring smooth transitions from our detox center to home or other appropriate care settings, helping to maintain continuity of care.
