Continuing Care Peer

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare organization offering a full continuum of care, including variety of programs throughout Arizona, Oklahoma, New Mexico, Oregon and District of Columbia. Founded in 1982 as a non-profit organization by members of the recovery community in Mesa, Arizona, Community Bridges Inc. has held as its principal mission “maintaining the dignity of human life” for more than 40 years.

Job Summary

The Continuing Care Peer is an individual with lived experience in recovery and who provides community-based outreach, support, navigation, and care coordination for individuals experiencing behavioral health challenges, substance use, homelessness, or crisis situations. This position provides peer support, outreach, care planning, and resource linkage both onsite and in the community. This Continuing Care Peer supports individuals during deflection encounters, sobering services, and transitions to appropriate levels of care, while ensuring followup, advocacy, and continuity of services

Skills/Requirements: 

  • High school diploma or GED required.
  • Minimum six months of recovery from substance use and/or mental health challenges.
  • Experience in outreach, peer support, homeless services, or behavioral health preferred.
  • Driver’s license (valid and in good standing) required.
  • Clear 39-month motor vehicle record required.
  • Peer Support Specialist certification from the Oregon Health Authority required or able to be obtained within 90 days of hire.
