• Design, implement, and revise individualized treatment plans for clients to address specific therapeutic goals, adapting approaches to suit varying client needs.
• Monitor client progress through regular assessments, providing comprehensive updates, and modifying treatment plans based on evolving client situations.
• Deliver immediate crisis intervention services when necessary, offering critical support during acute mental health episodes.
• Establish and nurture working relationships with families and community service providers, creating a robust support network that aids in client recovery.
• Maintain meticulous documentation of all client interactions, treatment methods employed, and outcomes achieved to ensure compliance and facilitate continuity of care.
• Engage in clinical meetings and training sessions to discuss case developments, share insights, and ensure adherence to best practices.
• Collaborate with other healthcare professionals to provide integrated care, contributing to multidisciplinary discussions that enhance client outcomes.

The client population served at Polaris includes individuals and families navigating complex emotional and psychological challenges often related to end-of-life issues. You will encounter clients facing grief, depression, anxiety, and adjustment disorders, particularly those within hospice care. • A Master’s degree in Social Work is required, demonstrating a solid foundational knowledge of social work principles and practices.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in California is mandatory for this position.
• Comprehensive understanding of psychological frameworks and therapeutic methodologies is essential.
• Exceptional interpersonal and communication skills, allowing for effective engagement with clients, families, and professional colleagues.
