• Conduct comprehensive client assessments via virtual meetings, evaluating social, emotional, and environmental factors that affect their wellbeing.
• Develop and implement individualized treatment plans, ensuring that plans are regularly updated in accordance with client progress and feedback.
• Advocate for client rights and facilitate access to necessary resources such as counseling services, community programs, and financial assistance.
• Collaborate closely with an interdisciplinary team of healthcare professionals to provide integrated care that addresses all aspects of the client's needs.
• Document all interactions and progress in compliance with state and federal regulations, ensuring that case notes are accurately maintained in the electronic health record system.

You will primarily work with a diverse population of patients receiving hospice care, many of whom face complex emotional and psychological challenges at the end of life. Understanding the nuances of grief, loss, and family dynamics is essential. Your role will be pivotal in providing support not only to patients but also to their families, helping them navigate this difficult time with dignity and compassion.

• A Bachelor's degree in Social Work from an accredited institution is required; a Master’s degree in Social Work (MSW) is preferred.
• Must hold a valid Licensed Clinical Social Worker (LCSW) license in California.
• Exceptional communication skills to effectively articulate complex concepts and empathize with clients' situations.
• Proven ability to work independently in a remote capacity while remaining committed to team goals and collective success.

• At least three years of experience working in social work, particularly in hospice, palliative care, or similar healthcare settings.
• Experience utilizing telehealth methods to deliver social work services.
• Proficiency in electronic health record (EHR) systems and other remote collaboration tools.
• Demonstrated ability to navigate social service systems and provide linkage to community resources.

This full-time position offers a competitive salary range of $115,000 to $130,000 per year, commensurate with experience.
