Working closely with educators, caregivers, and healthcare professionals, the Social Worker will engage in interdisciplinary teamwork to coordinate care and ensure a holistic approach to client support.

• Conduct initial and ongoing assessments of seniors' psychosocial needs, focusing on emotional, mental, and social factors that impact their well-being.
• Develop personalized care plans in collaboration with clients and their families, setting realistic goals that cater to individual preferences and life transitions.
• Provide counseling and emotional support to clients and their families, guiding them through various challenges, including health crises and family dynamics.
• Advocate for clients’ rights, ensuring they have access to necessary health services and community resources that promote optimal living conditions.
• Engage in regular communication with interdisciplinary teams, working alongside nurses, behavioral health specialists, and other professionals to enhance client care.
• Schedule and conduct regular visits to assess client well-being and living conditions, addressing any emerging safety concerns timely.
• Facilitate family discussions to address caregiving challenges, improve patient care outcomes, and strengthen communication among stakeholders.
• Educate clients and families on available resources, support networks, and programs that enhance social engagement and emotional support.
• Maintain accurate documentation of assessments, care plans, and client interactions in compliance with regulatory standards.
• Stay informed of best practices in social work and contribute to ongoing professional development initiatives.

This role primarily focuses on seniors, who often face complex psychosocial challenges related to aging, health issues, and life transitions. As an LCSW, you will work intensely to understand and address these senior clients’ needs, advocating for their well-being and enhancing their quality of life through tailored interventions.

• A Master’s Degree in Social Work (MSW) from an accredited institution is required.
• Active and valid state licensure as a Clinical Social Worker (LCSW).
• A minimum of two years of relevant experience in social work, particularly in geriatric or community settings.
• Strong understanding of social work principles, aging-related issues, and community resources.
• Exceptional assessment, counseling, and problem-solving abilities.
• Compassionate interpersonal skills, enabling effective engagement with clients and their families.
• Proficiency with electronic health records (EHR) systems and various computer applications.

• Prior experience working in a school-based setting is advantageous but not mandatory.
• Experience in crisis intervention and conflict resolution in family dynamics related to elder care.
• Familiarity with telehealth platforms and tools for remote service delivery.
