As a Licensed Clinical Social Worker at Support Wellness Healthcare Services, your key responsibilities will include:

• Leading psychoeducational and process-oriented therapy groups that empower clients to develop coping strategies and enhance their mental health.
• Conducting thorough clinical assessments to analyze patient needs and determine appropriate treatment plans.
• Initiating discharge planning on the day of admission for all patients and following through with comprehensive discharge sessions to ensure a seamless transition to continued care.
• Coordinating with referral sources and outpatient providers for optimal continuity of care throughout each stage of treatment.
• Facilitating patient transfers to higher levels of care when necessary, ensuring the highest quality of support.
• Managing crises effectively during therapy sessions and across the treatment milieu.
• Providing ongoing support to milieu staff as they interact with patients and their loved ones.
• Collaborating with the entire treatment team to maintain a commitment to clinical excellence and patient-centered care.
• Completing necessary documentation including initial assessments, SMART treatment plans, and clinical case notes in a timely manner.
• Evaluating patients for risk factors such as suicidality or potential danger to self or others, and addressing these concerns proactively.

You will work with a diverse client base that includes individuals from various backgrounds, ages, and socioeconomic statuses. • Proficiency in using Office 365 and Electronic Health Record systems.
