Key Responsibilities

• Conduct thorough initial assessments for clients seeking social work support and develop personalized treatment plans tailored to their unique needs.
• Engage in regular follow-up sessions to monitor client progress and adjust treatment plans as necessary, ensuring a responsive approach to evolving client needs.
• Advocate for clients by helping them access essential social services, mental health resources, and community support systems.
• Utilize evidence-based practices to implement intervention strategies, including crisis intervention and emotional support provisions.
• Collaborate with multidisciplinary teams, including psychiatrists, psychologists, and other healthcare professionals, to provide comprehensive care and case management.
• Maintain accurate and confidential documentation in compliance with state and federal regulations, ensuring that client records reflect ongoing engagement and treatment effectiveness.

Patient Population / Client Focus

You will be primarily serving a diverse population that includes individuals struggling with mental health challenges, such as depression, anxiety, and trauma-related disorders. Our client demographic encompasses various age groups, cultural backgrounds, and socioeconomic statuses, requiring a tailored approach to each individual case.

Qualifications

• Bachelor's degree in Social Work from an accredited institution.
• Active Licensed Clinical Social Worker (LCSW) certification in California.
• A minimum of 3 years of relevant social work experience, particularly within a mental health or community health setting.
• Exceptional communication skills, with a strong capacity for empathy and active listening.
• Familiarity with telehealth technology and an ability to navigate digital platforms effectively.

Preferred Experience

• Experience working with diverse populations and an understanding of cultural competency in social work practices.
• Proficiency in evidence-based therapeutic interventions, including cognitive-behavioral therapy (CBT) and solution-focused brief therapy (SFBT).
• Prior experience in a community health setting, demonstrating knowledge of local resources and service networks.

Compensation and Benefits
