• Conduct individual and family counseling sessions through video conferencing platforms, ensuring a comfortable environment for clients to share their thoughts and feelings.
• Perform comprehensive psychosocial assessments to evaluate clients' needs and develop effective, personalized treatment plans that address their unique situations.
• Facilitate therapeutic interventions that emphasize coping strategies, grief counseling, and support for end-of-life issues.
• Advocate for clients by providing resources and support that help them achieve their personal goals related to mental health and overall well-being.
• Collaborate with an interdisciplinary team, including nurses and physicians, to coordinate care and improve patient outcomes.
• Keep accurate and timely documentation of patient interactions, treatment plans, and progress in compliance with organizational policies and health regulations.
• Participate in team meetings and case consultations to discuss complex cases and share insights for enhanced care delivery.

Your primary focus will be on clients in a hospice setting, who may be dealing with terminal illnesses or facing end-of-life transitions. This population often requires specialized support to navigate emotional distress, grief, and existential concerns. Your role will be pivotal in helping these individuals and their families cope with significant life changes, promoting emotional resilience during challenging times.

• A Master’s degree in Social Work from an accredited institution is required.
• A valid state license as a Licensed Independent Clinical Social Worker (LICSW) without any disciplinary actions.
• Proven experience in clinical social work practices, particularly in assessment, diagnosis, and treatment planning.
• Strong interpersonal and communication skills to effectively build rapport and establish trust with clients from diverse backgrounds.
• Proficiency in utilizing digital tools and platforms for remote therapy, including electronic health records and telehealth technologies.

• Experience working specifically with hospice or palliative care populations is highly desirable.
• Familiarity with grief counseling and crisis intervention techniques.
• A background in multidisciplinary teamwork within healthcare environments will be beneficial.
