The role of an LCPC at Crystal Lake is primarily remote, allowing for flexible working conditions. As a participant in a community-oriented clinical environment, you will play a vital role in a team setting, contributing your expertise in mental health while collaborating with other healthcare providers. Your home office can be equipped as needed to provide the best remote care experience.

As a Licensed Clinical Professional Counselor, your responsibilities will include:

• Conducting intakes focusing on diagnostic and functional evaluations, followed by making informed recommendations for treatment plans to primary care providers.
• Providing brief, focused interventions for patients requiring mental health services within the integrated healthcare framework.
• Assisting primary care providers in recognizing and managing psychosocial issues while contributing to a cohesive primary care team.
• Monitoring patient progress and coordinating with other treatment providers, both within and outside the primary care setting, to enhance patient care.
• Documenting patient progress meticulously and preparing clinical records that support treatment plans and outcomes.
• Educating patients on self-management skills and lifestyle changes to support long-term well-being.
• Working in conjunction with Clinic Management to evaluate the effectiveness of behavioral services and to identify areas for improvement.

Your role will involve working with individuals, families, and children from various backgrounds, understanding their unique circumstances and providing tailored therapeutic interventions. Special focus will be given to underserved populations, ensuring that our services are accessible and meet the varied needs of our community.

The ideal candidate will possess the following qualifications:

• A Master’s Degree in Clinical Counseling or a related field.
• A current and active Licensed Clinical Professional Counselor (LCPC) designation.
• Extensive knowledge in areas such as mental health assessment, treatment planning, and psychotherapeutic techniques.
• Proficiency in English is essential; bilingual skills in Spanish are preferred but not required.
• Strong communication skills, with the ability to establish rapport and maintain productive relationships with clients and fellow team members.
• A commitment to ethical practices and adherence to the Code of Ethical Conduct for Clinical Professionals.

Experience working within Federally Qualified Health Centers (FQHC) or integrated healthcare settings is highly desirable. Familiarity with DSM-IV/5 diagnostics, development psychopathology, and intervention strategies for mental health conditions will give candidates a competitive edge.
