Our technology platforms streamline communication with your patients and team members, ensuring a cohesive and efficient workflow.

• Provide telephonic or virtual educational support to patients, addressing their mental health needs and concerns.
• Serve as the primary contact for patients, liaising with other healthcare professionals to coordinate care effectively.
• Assess each patient's individual support needs and preferences to create a personalized care experience that facilitates their treatment journey.
• Educate patients on program policies and procedures while meticulously documenting interactions in the clinical chart.
• Maintain accurate records and progress notes, ensuring the clinical chart includes comprehensive information on each patient's status and treatment updates.

The ideal candidate will work with a diverse clientele, including individuals from varying socio-economic backgrounds and those facing unique mental health challenges. You will play an integral role in fostering a supportive and healing environment, assisting clients in navigating their pathways to recovery.

• A Master's degree in Social Work, Counseling, or a related field is required.
• Current licensure as a Licensed Clinical Social Worker (LCSW) in the state of California is essential.
• A minimum of three years of experience in a counseling role is mandatory, demonstrating a strong understanding of mental health practices.
• Exceptional communication skills are necessary, both written and verbal, to effectively interact with patients and healthcare professionals.
• The ability to adapt and thrive in a competitive, fast-paced environment is required.
• Intermediate to advanced proficiency in Microsoft Office applications, particularly Word, Outlook, and Excel, is preferred.

Experience working within community health settings or private practice, particularly with diverse populations, will be beneficial. Familiarity with telehealth platforms and electronic health record systems is a plus, as these tools will be integral to your role.
