Work Environment

This role is entirely remote, which allows for flexibility and work-life balance. Our hospice center offers a supportive virtual environment that facilitates meaningful interactions through telehealth sessions. Key Responsibilities

• Provide virtual or telephonic educational support to patients diagnosed with various mental health conditions, ensuring they receive guidance tailored to their unique needs.
• Serve as the primary point of contact for both patients and healthcare professionals, establishing strong lines of communication to facilitate seamless care transitions.
• Assess and understand each patient's specific support requirements while incorporating their preferred modes of interaction to enhance the overall patient experience.
• Educate patients on program policies and procedures, documenting all necessary information in clinical charts to maintain accurate patient records.
• Regularly update progress notes and ensure that clinical documentation fulfills all regulatory requirements and reflects real-time patient status.

Patient Population / Client Focus

In this role, you will work primarily with patients enrolled in hospice care, a sensitive field that requires empathy and understanding. Many patients you will support may be experiencing complex emotional and psychological challenges related to their health status. Your expertise will be essential in promoting their mental and emotional wellbeing during what may be one of the most challenging periods of their lives. Providing them with compassionate support will contribute significantly to their holistic experience of care.

Qualifications

• A Master's degree in Social Work (MSW), Counseling, or a related field is required.
• Active and valid Licensed Clinical Social Worker (LCSW) certification in the state of California.
• A minimum of three years of relevant work experience in clinical counseling is essential.
• Excellent verbal and written communication skills, allowing for clear and effective engagement with patients and multidisciplinary teams.
• A proactive mindset with the ability to adapt to a dynamic and competitive remote work environment.
• Intermediate to advanced proficiency in computer skills, especially with Microsoft Office applications such as Word, Excel, and Outlook.

Preferred Experience

• Previous experience working in hospice, palliative care, or similar settings is highly desirable.
• Familiarity with telehealth technologies and best practices to engage effectively with patients remotely.
• Experience in developing and implementing individualized care plans tailored to patients' needs.

Compensation and Benefits

The salary range for this position is competitive, with an annual compensation of USD 120,000 to 130,000. Our comprehensive benefits package includes:
