Uplift Hub Social Work is actively seeking a compassionate and skilled Licensed Clinical Social Worker (LCSW) to join our dynamic team in a full-time remote capacity. Work Environment

This role offers a remote working environment, ensuring you can provide support and services in a flexible setting while still being connected with your colleagues. Our Hospice Center is committed to fostering a supportive online community where mental health professionals can collaborate, share insights, and engage in professional development. You will be a part of a multidisciplinary team that includes psychiatrists, nurses, and other mental health specialists, working together to provide holistic care.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments to evaluate the mental health needs of clients, allowing the development of tailored intervention strategies.
  • Facilitate individual, family, and group therapy sessions, employing evidence-based therapeutic interventions such as cognitive-behavioral therapy and motivational interviewing to address unique challenges faced by clients.
  • Establish and monitor individualized treatment plans, ensuring that goals align with clients' needs and progress is regularly assessed.
  • Provide crisis intervention services, which may include conducting risk assessments and developing safety plans to support clients experiencing acute distress.
  • Collaborate with a diverse team of professionals to ensure coordinated, empathetic, and effective care for all clients.
  • Advocate for clients' rights and assist them in accessing community resources, thereby enhancing their treatment experience and outcomes.
  • Engage in outreach initiatives to promote mental health awareness, combat stigma, and build community relationships that facilitate greater access to care.
Patient Population / Client Focus

In this role, you will work with a wide variety of individuals and families, providing crucial support to those facing mental health challenges, including depression, anxiety, grief, and trauma. Qualifications
  • A Master's degree in Social Work from an accredited program.
  • Current state licensure as a Licensed Clinical Social Worker (LCSW).

Preferred Experience

Compensation and Benefits
