Uplift Hub Social Work is seeking an experienced Senior Licensed Clinical Social Worker (LCSW) to join our dedicated team in providing high-quality mental health support to our clients. Work Environment

This position is fully remote, allowing you to maintain an efficient work-life balance while supporting clients from the comfort of your home. Our Hospice Center environment emphasizes compassion and holistic care, working closely with individuals at crucial stages of their mental health journeys.

Key Responsibilities

• Conduct comprehensive psychosocial assessments to understand clients' mental health issues, strengths, and challenges.
• Deliver tailored individual, family, and group therapy, employing evidence-based practices such as cognitive-behavioral therapy (CBT) and motivational interviewing.
• Develop, implement, and monitor individualized treatment plans that align with clients' goals and progress, ensuring culturally competent strategies are utilized.
• Engage in crisis intervention, providing immediate support to clients in distress, conducting risk assessments, and drafting comprehensive safety plans.
• Collaborate with a multidisciplinary team, including psychiatrists and nursing staff, to coordinate comprehensive care pathways for clients.
• Advocate for clients’ rights and connect them with vital community resources, while providing necessary case management assistance.
• Participate in community outreach initiatives designed to raise awareness about mental health issues and reduce stigma.

Patient Population / Client Focus

As a Senior LCSW at Uplift Hub Social Work, you will work with a diverse client population, including individuals with varying socio-economic statuses, cultural backgrounds, and mental health challenges. Your role will involve providing compassionate support to clients navigating crises, chronic mental illnesses, and those in need of transitional or ongoing counseling within a hospice care setting.

Qualifications

• Master's Degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• Strong understanding of mental health principles and practices, with a focus on evidence-based methodologies.
• Exceptional interpersonal skills with a demonstrated ability to communicate effectively and empathetically with clients, families, and colleagues.
• Dedication to cultural competence and the ability to work sensitively with diverse populations.

Preferred Experience

• Minimum of two years’ experience in a community mental health setting or similar environment.
• Experience in hospice or end-of-life care services is a significant advantage.
• Familiarity with crisis intervention strategies and case management practices.

Compensation and Benefits
