• Conduct thorough psychosocial assessments to understand patients' mental and emotional health needs, concerns, and strengths.
• Facilitate individual, family, and group therapy sessions, employing evidence-based therapeutic methodologies such as Cognitive Behavioral Therapy (CBT), Solution-Focused Therapy, and Trauma-Informed Care.
• Develop personalized treatment plans that set measurable goals and interventions tailored to each patient’s unique situation, aligning with hospice care requirements.
• Coordinate with other healthcare professionals, including nurses and medical doctors, to ensure seamless care delivery and consistent messaging regarding patient needs.
• Engage in crisis intervention and risk assessment strategies, providing immediate support when necessary to enhance patient stability.
• Document therapy sessions and treatment plans accurately, adhering to ethical standards and compliance regulations.
• Lead psychoeducation workshops and support groups aimed at enhancing coping strategies for patients and family members navigating grief and loss.

This role focuses on working with patients receiving hospice care, which includes end-of-life individuals and their families. You will provide emotional support and counseling to help them cope with the psychological impacts of terminal illness, grief, and the dying process. The breadth of our services allows us to support a diverse clientele, emphasizing personalized care to meet the unique needs at this crucial time in their lives.

• A Master's degree in Social Work from an accredited institution.
• Current active licensure as a Licensed Clinical Social Worker (LCSW) in the state of Texas.
• Minimum of two years of experience in a mental health or outpatient treatment setting, ideally with a focus on hospice or end-of-life care.
• Strong familiarity with evidence-based therapies and treatment modalities, demonstrating a commitment to best practices in mental health.
• Exceptional interpersonal, communication, and assessment skills, allowing for effective interactions with patients and their families.

• Experience working in hospice, palliative care, or a healthcare setting focusing on mental health support during critical life transitions.
• Knowledge of grief counseling and bereavement support strategies, which are crucial to this role.
• Experience conducting group therapy or educational workshops related to mental health.
