Your key responsibilities will encompass a diverse range of vital services, including:

• Performing comprehensive psychosocial assessments to evaluate patients' emotional, social, and financial needs.
• Delivering individual and group counseling sessions tailored to help patients cope with adjustment issues, grief, and anxiety associated with illness.
• Coordinating with community organizations to secure post-discharge resources and services, ensuring seamless transitions from hospice care to home or community settings.
• Providing education to patients and families about treatment options, psychosocial resources, and emotional coping strategies.
• Collaborating with interdisciplinary teams to improve patient outcomes by addressing psychosocial factors that impact health.
• Participating actively in hospital committees to contribute expertise on social work practices and patient experiences.
• Documenting all interventions accurately and timely to comply with institutional policies and legal standards.
• Offering crisis intervention and support to patients and families in times of emotional distress.
• Developing and implementing effective discharge plans that connect patients with appropriate community resources.

This role focuses on serving patients in a hospice setting, where understanding the intricacies of end-of-life care is paramount. You will work with individuals facing life-limiting illnesses, offering them and their families the emotional support necessary to cope with terminal diagnoses, alongside providing assistance ensuring dignity and quality of life through compassionate care.

To be considered for this role, candidates must possess the following qualifications:

• A Master’s degree in Social Work from an accredited program.
• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).
• Expertise in psychosocial assessment and intervention tailored to varied patient demographics.
• Proficient knowledge of mental health terminology, adjustment disorders, and crisis intervention strategies.
• Exceptional communication and interpersonal skills to effectively connect and support patients and their families.

While the minimum requirement is essential, the following experiences are preferred:

• At least two years of experience working in a healthcare or hospital environment.
• Experience in hospice care or with palliative patient populations.
• Demonstrated ability to collaborate across interdisciplinary teams to enhance patient care.

This position offers a competitive salary range of USD 70,000 to 100,000 annually, commensurate with experience and expertise.
