Hospice Registered Nurse (RN) - Ocean County, NJ

Job Summary:

Swan Hospice is seeking a compassionate and experienced Hospice Registered Nurse (RN) to join our team of dedicated caregivers. As a Hospice RN with Swan Hospice, you will play a vital role in providing supportive, holistic care to terminally ill adult patients and their families. Our team is committed to delivering personalized end-of-life care plans that are implemented with dedication, skill, and sensitivity.

Responsibilities:

  • Provide high-quality, patient-centered care to terminally ill adult patients, focusing on comfort, dignity, and quality of life

  • Collaborate with interdisciplinary teams to develop and implement individualized care plans, addressing physical, emotional, and spiritual needs

  • Conduct thorough assessments, develop and implement patient care plans, and coordinate care with other healthcare professionals

  • Educate patients, families, and caregivers on disease management, symptom control, and end-of-life care

  • Provide emotional support and counseling to patients and their loved ones

  • Document patient care activities, assessments, and outcomes in accordance with company policies and regulations
