Hospice RN at Grand County Hospice At Grand County Hospice, we are committed to providing compassionate, high-quality end-of-life care to our patients and their families. As a leader in hospice care, we believe in treating every patient with dignity and respect in their preferred place of care - usually in their home. We are seeking a dedicated and compassionate Hospice Registered Nurse (RN) to join our interdisciplinary team. The RN will provide skilled nursing care and support to terminally ill patients and their families in accordance with the patient's plan of care and hospice philosophy. Key Responsibilities: โ€ข Perform comprehensive assessments of patients' needs and coordinate individualized care plans. โ€ข Provide pain and symptom management consistent with the hospice plan of care. โ€ข Educate and support patients and families regarding the disease process, self-care techniques, end-of-life issues, and the hospice philosophy. โ€ข Collaborate with the interdisciplinary team including physicians, social workers, chaplains and volunteers. โ€ข Participate in on-call rotation โ€ข Maintain accurate and timely documentation in compliance with state, federal, and organizational policies. Qualifications: โ€ข Current Registered Nurse (RN) license in the our client Utah or compact state RN license. โ€ข Minimum of 3 year of nursing experience, preferably in hospice, home health or palliative care. โ€ข Strong clinical assessment skills. โ€ข Excellent communication, interpersonal, and organizational skills. โ€ข Compassionate demeanor and ability to support patients and families during difficult times. โ€ข Valid driver's license and reliable transportation required for home visits. Mileage reimbursement $.70 per mile. โ€ข Current BLS Certification โ€ข On-Call 1-3 shifts per week, to include 3 weekends during the 13 week contract depending on staffing levels.
