Job Summary
We are seeking aย  Home Careย RNย to join our team. In this role, you will be the DONย  primarily be creating and managing a plan of care for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems.ย 
Responsibilitiesย 
  • Who has held a position of a supervisor in home health care for at least two yrsย  and able to prepare a comprehensive care plan.
  • ย Coordinate andย  superviseย  other colleagues to assess,ย  implement and evaluate staff performance
  • Able to do admin work and also comply with accreditation processย 
  • Document any changes in patient conditions and able to do incidentย  report documentationย ย 
  • Maintain accurate records of patient care and discuss observations with the managementย  ย 
Qualifications
  • Currently licensed Registered Nurse (RN) in state of employmentย 
  • Previous experience as a Home Care RN with two YRS Experience wouldย  be preferred
  • Excellent in communication skills, interpersonal andย  client care skills
  • Valid driverโ€™s license ,Physicals, Xray, a copy of previous background checks will be a plusย 
  • First aid/CPR certification /malpractice insurance is requiredย
