As the Home Care RN for ComForCare Chattanooga, you will play a central role in ensuring that every client receives safe, high-quality, person-centered care. This position combines clinical expertise with relationship-building, care planning, and caregiver oversight. You will be the clinical face of the agency, supporting clients, families, and caregivers with compassion and professionalism.
What You'll Do
  • Complete comprehensive in-home care assessments to evaluate medical needs, functional abilities, safety concerns, and overall well-being.
  • Build and maintain strong, trusting relationships with clients and families to support long-term satisfaction and continuity of care.
  • Develop individualized care plans that reflect client needs, preferences, and physician recommendations, updating them as conditions change.
  • Conduct caregiver skills assessments, including hands-on competency checks for personal care, mobility assistance, infection control, and condition-specific tasks.
  • Provide coaching, training, and clinical guidance to caregivers to ensure they deliver care safely and confidently.
  • Reassess clients on a regular schedule and adjust care plans accordingly.
  • Document assessments, care plans, and caregiver evaluations accurately and in compliance with ComForCare standards and state regulations.
  • Collaborate closely with the owner and office team to ensure seamless service delivery and exceptional client outcomes.

Qualifications
  • Active RN license in the state of Tennessee.
  • At least one year of nursing experience; home care, geriatric, or community health experience is a plus.
  • Strong assessment and care-planning skills.
  • Ability to evaluate caregiver competency and provide constructive, supportive feedback.
  • Excellent communication and interpersonal skills.
  • Reliable transportation for local travel to client homes.
  • Able to pass background check and drug test according to company policy.

Preferred Experience
  • Experience in home care or private duty settings.
  • Background in caregiver training, supervision, or clinical education.
  • Familiarity with home care regulations and electronic documentation systems.
