• Conduct thorough, holistic assessments of patients' psychosocial needs, ensuring a comprehensive understanding of their circumstances and support requirements.
• Develop and implement tailored treatment plans that address the unique needs of each patient, incorporating evidence-based therapeutic interventions to promote emotional well-being.
• Maintain accurate and confidential patient records in accordance with regulatory standards, utilizing electronic health record (EHR) systems designed to facilitate integrated care.
• Collaborate closely with a multidisciplinary team, including nursing staff, medical professionals, and other social workers, to provide coordinated care that respects each patient’s preferences and decisions.
• Engage in continuous professional development by staying informed on the latest research, therapies, and practices in social work and hospice care.

You will engage primarily with patients experiencing terminal illnesses, focusing on conditions such as cancer, heart disease, and neurological disorders. • A Master's degree in Social Work from an accredited institution is required.
