• Conduct comprehensive psychosocial assessments to identify the mental health concerns, strengths, and resources of clients within a hospice care setting.
• Deliver evidence-based individual, family, and group therapy sessions focused on grief, loss, and adjustment to life-threatening illness.
• Implement treatment modalities like cognitive-behavioral therapy, solution-focused therapy, and trauma-informed interventions tailored to hospice care.
• Develop individualized treatment plans incorporating client-specific goals, therapeutic interventions, and measurable outcomes.
• Collaborate with healthcare professionals—such as medical doctors, nurses, and other social workers—to provide integrated and coordinated patient care.
• Engage in crisis intervention when necessary, including risk assessment and safety planning for clients and their families.
• Facilitate psychoeducation classes, group therapy sessions, and support groups designed to promote coping and resilience among patients and families.
• Maintain thorough and accurate documentation of client interactions, treatment plans, and outcomes in compliance with ethical and legal standards.

The ideal candidate will work primarily with hospice patients and their families, addressing complex emotional and psychological needs in the final stages of life. Your compassionate approach and therapeutic skills will play a crucial role in enhancing the quality of life for individuals experiencing significant transitions related to terminal illness, providing support throughout the grieving process and helping families navigate difficult conversations and decisions.

• A master’s degree in Social Work from an accredited program.
• Active state licensure as a Licensed Clinical Social Worker (LCSW).
• A minimum of two years of clinical experience, particularly in mental health or outpatient settings, with a strong emphasis on hospice or palliative care preferred.
• Sound knowledge of evidence-based therapeutic modalities and a commitment to continuous professional development in therapeutic practices.
• Exceptional interpersonal, communication, and assessment skills, with the ability to establish rapport with clients of diverse backgrounds.

• Experience in hospice, palliative care, or end-of-life counseling settings.
• Familiarity with multidisciplinary healthcare teams and working within collaborative environments.
• Training in trauma-informed care and grief counseling techniques.

The salary for this full-time role ranges from $80,000 to $100,000 annually, commensurate with experience and qualifications. In addition, our comprehensive benefits package includes:
