• Conduct comprehensive assessments related to the psychosocial needs of hospice patients, identifying both emotional and social factors impacting their care.

• Implement evidence-based therapeutic interventions to assist patients and their families in coping with grief, loss, and other emotional challenges.
• Maintain meticulous clinical documentation that meets ethical, legal, and organizational standards.
• Collaborate effectively with the multidisciplinary team, including physicians, nurses, and spiritual care providers, to ensure holistic patient care.
• Facilitate support groups for patients and their families, providing education on coping strategies and available resources.
• Stay abreast of advancements in social work practices and familiarize yourself with local and national guidelines affecting hospice care.

The patient population you will serve includes terminally ill individuals and their families who require specialized support as they navigate complex emotional and practical challenges. • A Master’s degree in Social Work from an accredited university.
• A valid LCSW license in the state of Texas, ensuring compliance with state regulations.

• Strong interpersonal communication skills with an emphasis on empathy and understanding.
