• Conduct thorough psychosocial assessments to understand the individual needs of each patient, evaluating their emotional, social, and spiritual needs.
• Develop personalized treatment plans based on assessment findings and evidence-based practices, ensuring tailored therapeutic interventions are delivered.
• Implement solution-focused therapy techniques to address issues related to trauma, anxiety, and depression, particularly in patients and families facing end-of-life challenges.
• Maintain meticulous and confidential clinical records to ensure compliance with both organizational policies and state regulations.
• Collaborate with an interdisciplinary team, including nurses, doctors, and other social workers, to enhance patient care and ensure cohesive treatment strategies.
• Engage in ongoing professional development to remain current with research and advances in social work practices and hospice care methodologies.

You will primarily work with individuals who are facing terminal illnesses, providing emotional and psychological support throughout their treatment process. This role involves working closely with patients from diverse backgrounds who may be experiencing feelings of fear, isolation, and anxiety related to their conditions. Delivering empathetic care requires a deep understanding of the complexities involved in end-of-life situations, ensuring that clients feel valued and understood during this critical phase of their lives.

• A Master's degree in Social Work from an accredited institution.
• Possession of an active LCSW license in the state of Texas.
