Work Environment

This position is fully remote, allowing for a flexible schedule and the ability to provide care from the comfort of your home. Key Responsibilities
  • Act as a liaison between clients, families, and outpatient healthcare providers to facilitate a comprehensive support network.
  • Engage in proactive communication with insurance companies to handle pre-certifications and manage utilization effectively, thereby ensuring clients receive the care they need in a timely manner.
  • Participate actively in discharge planning and aftercare coordination to support clients transitioning from inpatient settings back to community services.
  • Provide therapeutic meal support, guiding clients to develop and model healthy relationships with food, enhancing psychological and physical well-being.
  • Deliver personalized care that respects and addresses the unique needs of each client based on their distinct intersecting identities, ensuring they feel safe, heard, and understood.
Patient Population / Client Focus

This role predominantly focuses on working with older adults, a demographic that often faces complex social and emotional challenges. You will use your clinical expertise to address the unique circumstances of aging individuals, which may include issues related to chronic illness, isolation, loss, and identity. Qualifications
  • A Master's degree in Social Work from an accredited institution.
  • A valid LCSW license in the state of Kansas is mandatory.
