Position Overview

Work Environment

This position is fully remote, allowing for a flexible schedule and the ability to provide care from the comfort of your home. Although you will primarily be working independently, you will remain an integral part of our multidisciplinary team, which includes therapists, case managers, and collaborative partners, all working towards the same goal of improving client mental health outcomes.

Key Responsibilities

• Act as a liaison between clients, families, and outpatient healthcare providers to facilitate a comprehensive support network.
• Engage in proactive communication with insurance companies to handle pre-certifications and manage utilization effectively, thereby ensuring clients receive the care they need in a timely manner.
• Participate actively in discharge planning and aftercare coordination to support clients transitioning from inpatient settings back to community services.
• Provide therapeutic meal support, guiding clients to develop and model healthy relationships with food, enhancing psychological and physical well-being.
• Deliver personalized care that respects and addresses the unique needs of each client based on their distinct intersecting identities, ensuring they feel safe, heard, and understood.

Patient Population / Client Focus

This role predominantly focuses on working with older adults, a demographic that often faces complex social and emotional challenges. You will use your clinical expertise to address the unique circumstances of aging individuals, which may include issues related to chronic illness, isolation, loss, and identity. Our clients come from diverse backgrounds, offering you the opportunity to leverage your skills in a rich and varied therapeutic context.

Qualifications

• A Master's degree in Social Work from an accredited institution.
• A valid LCSW license in the state of Kansas is mandatory.
• Demonstrated experience in providing psychosocial support to older adults is essential.
• Strong clinical and diagnostic skills, equipped to assess needs and implement appropriate interventions.
• Excellent communication and interpersonal relationship-building skills to foster trusting connections with clients, families, and team members.
• A self-motivated attitude, with the ability to work effectively both independently and as part of a cohesive team.

Preferred Experience

• Experience in hospice or palliative care settings is highly desirable.
• Familiarity with electronic health record (EHR) systems and teletherapy platforms.
• Additional certifications in gerontology or a related field would be advantageous.
