Position Summary

The Care Coordinator Lead oversees daily care coordination operations for the Orlando/Central Florida market. This role combines hands-on care coordination with team leadership, mentoring new Care Coordinators, and serving as the primary clinical operations liaison between providers, external home health agencies, labs, and the central operations team. The ideal candidate is an experienced RN with strong organizational skills who thrives in a fast-paced, remote healthcare environment.

Essential ResponsibilitiesTeam Leadership
  • Supervise and mentor a team of Care Coordinators, including onboarding, training, and ongoing performance support
  • Serve as first point of escalation for CC team questions on referrals, prior authorizations, DME orders, and clinical workflows
  • Coordinate daily huddles and team coverage schedules (breaks, PTO, on-call rotation)
  • Monitor team workload and redistribute tasks to maintain patient care standards
Care Coordination
  • Process and route incoming referrals, lab orders, prior authorizations, and home health orders to appropriate providers for review and signature
  • Coordinate medication refills, lab result routing, and follow-up scheduling with NPs and physicians
  • Manage relationships with external home health agencies (VitalCaring, CenterWell, Amedisys, Patients First, etc.) for order signatures, POC coordination, and patient referrals
  • Route lab specimens and results through partner laboratories (LabFlorida, RapidXUS, DBS Laboratories)
  • Coordinate DME orders (wheelchairs, wound vacs, supplies) with vendors and document appropriately
  • Maintain accurate patient records in OA (EHR), including chart filing, plan of care documentation, and task management
Clinical Operations
  • Triage patient calls and route to appropriate Care Coordinators or providers within the Orlando Call Center CC Command Center (Teams)
  • Coordinate with Patient Intake (Hicham Moro's team) on appointment task workflows and call routing protocols
  • Interface with credentialing and billing teams on provider enrollment and claims-related patient questions
  • Support CCM, RPM, and BHI billing workflows by ensuring proper documentation and care plan completion
  • Manage provider-specific communication channels (Teams) for real-time clinical coordination
Communication
  • Serve as primary point of contact for external healthcare partners in Central Florida
  • Relay voicemails, patient inquiries, and clinical messages between providers and patients/families
  • Participate in FL team check-ins and operational meetings
  • Escalate staffing concerns, workflow issues, or patient care risks to leadership
Required Qualifications
  • Active RN license in the State of Florida (compact/multistate accepted)
  • 3+ years of care coordination, case management, or clinical operations experience
  • 1+ year of team lead or supervisory experience in a healthcare setting
  • Experience with electronic health records (EHR/EMR) systems
  • Proficiency with Microsoft Teams, Outlook, and standard office applications
  • Strong knowledge of Medicare/Medicaid billing, CCM/RPM/BHI services, and home health order workflows
  • Excellent organizational skills with ability to manage multiple patient cases and team members simultaneously
Preferred Qualifications
  • Experience in home health, primary care, or mobile/in-home healthcare delivery
  • Bilingual English/Spanish (significant Spanish-speaking patient population in Central Florida)
  • Familiarity with prior authorization processes, Cover My Meds, and DME coordination
  • Experience with lab ordering platforms (LabFlorida, RapidXUS, Office Ally)
  • Prior experience working with acquired or transitioning healthcare practices
  • BSN preferred
Work Environment
  • Remote position based in Central Florida (Orlando metro area preferred)
  • Must be available during standard business hours (8:00 AM - 5:00 PM ET)
  • Occasional on-call or weekend coverage may be required
  • High-volume communication environment (Teams, email, phone)
  • ~15+ years of remote healthcare experience is the benchmark for this market
Physical Requirements
  • Prolonged periods sitting at a desk and working on a computer
  • Must be able to communicate clearly via phone and video conferencing

Pay Rate: $75,000- $85,000
