Mind Restoration Psychotherapy Center is actively seeking a dedicated and experienced Licensed Clinical Social Worker (LCSW) to join our team in a full-time remote capacity. This position offers the flexibility of remote work, allowing you to engage with clients from the comfort of your own home while maintaining a strong connection to our organization and the community. Despite the remote setting, you will collaborate closely with a multi-disciplinary team of health professionals to ensure comprehensive care and service delivery.

• Plan, coordinate, and implement personalized support packages aimed at helping clients navigate their challenges and overcome dependencies.
• Conduct thorough interviews and assessments of clients' current conditions, needs, strengths, and areas for growth, ensuring individualized care.
• Develop tailored, measurable goals for each client, thoughtfully addressing their situation and needs as a cohesive case.
• Monitor clients’ progress, continuously evaluating their treatment plans and making necessary adjustments to enhance outcomes.
• Provide clients with expert information and counseling regarding viable options to support their journey toward wellness.
• Maintain organized, accurate client records and documentation that reflect each individual's status and progress.
• Participate in case conferences, collaborating with other professionals, and if needed, provide evidence in court to advocate for client needs.
• Act as a key worker for assigned cases, engaging with multidisciplinary teams to ensure a holistic approach to client care.
• Refer clients to additional community services that can assist them in their recovery and overall improvement of quality of life.

Your primary focus will be working with diverse individual clients at the hospice center who are dealing with complex emotional and psychological issues related to their health transitions. This may include those experiencing grief, loss, or chronic illness. Providing a safe space for these clients to express their feelings and challenges will be central to your work, enabling you to tailor interventions that support their unique experiences.

• A Bachelor’s Degree in Social Work (BSW) or a related field is required. A Master’s Degree in Social Work (MSW), Psychology, or Sociology is preferred.
• A valid license as a Clinical Social Worker (LCSW) in the state of Colorado.
• Demonstrated experience as a social worker, with a proven track record in client advocacy and support.
• Strong understanding of social work theories and practices, with the ability to apply them effectively in client situations.
• Exceptional skills in building and maintaining professional therapeutic relationships with clients and colleagues.
• A high level of emotional intelligence, with social perceptiveness and empathy being critical to the role.
• Strong resilience and the capacity to assess client situations effectively, employing sound judgment in decision-making.
• Willingness to undergo background checks in compliance with organizational policies.

• Experience in hospice care or working with clients facing end-of-life issues is highly desirable.
• Familiarity with electronic health record (EHR) systems and the documentation requirements surrounding client care.
• Ability to engage with a diverse population, showcasing excellent communication skills across various contexts.
