This position operates in a remote work setting, allowing flexibility while ensuring a collaborative approach to patient care. You will be part of a dynamic team within a Community Health Clinic framework, working with various stakeholders to deliver integrated mental health services. • Deliver clinical services directly to clients experiencing major mental illness or severe emotional disorders through both in-office and community-based interactions.
• Engage in effective team participation to achieve shared objectives, promoting an environment of collaboration and support.
• Carry your own caseload, including assessments and therapy sessions—individual, group, and family—that are aligned with the patients' presenting issues.
• Conduct thorough, evidence-based screenings and assessments, facilitating referrals for more extensive evaluations as needed.
• Develop and implement integrated care plans that ensure clients receive seamless access to necessary support services, consistently exchanging information with clients, their families, and diverse care providers.
• Monitor the treatment process, including client meetings, admission criteria, and discharge plans to ensure appropriate service delivery.
• Provide crisis intervention as necessary, demonstrating sound judgment and a commitment to client safety.
• Contribute to quality improvement initiatives by participating in committees that focus on enhancing service delivery.
• Complete timely documentation, including reports and data submissions, to support operational integrity.
• Participate in on-call coverage as assigned to ensure 24/7 availability for client emergencies.

This role is critical for addressing the needs of a diverse patient population, particularly those located in under-resourced areas. The clients you will serve may include individuals with complex mental health needs, requiring a nuanced understanding of various cultural, economic, and social factors. You will be instrumental in creating a supportive space where clients feel empowered to seek and receive the help they need.

• A Bachelor’s Degree in Social Work or a related field is required; an LCSW license is essential.
• Expertise in assessment, diagnosis, treatment of mental health disorders, and crisis management.
• Impeccable verbal and written communication skills to convey important information effectively to clients and colleagues.
• Demonstrated organizational skills, with the ability to manage competing priorities while maintaining high-quality care.
• Ability to demonstrate cultural competence by effectively working with diverse populations and adapting to various interpersonal styles.

• Prior experience in a community health or similar environment is highly desirable.
• Experience with integrated care models and multidisciplinary teams will be an asset.
• Knowledge of local resources and support systems available to serve clients effectively.
