Our platform is designed to empower large provider groups and health systems to succeed in value-based care contracts with payers by improving patient outcomes and reducing cost of care through better coordination, analytics, and disease management. The Role
The Director of Clinical Operations will lead the development, refinement, and execution of Karoo's clinical care model. This leader will translate clinical strategy into scalable workflows, protocols, staffing models, and operational playbooks to ensure high-quality, compliant, and efficient care delivery across markets.
This role is ideal for a clinically trained operator with cardiology experience who understands value-based care and thrives in early-stage environments where building and iterating are core to the job.
This role will directly shape how care is delivered at Karoo, from the patient experience to clinical outcomes to financial performance under value-based contracts.
What You'll Do
Clinical Model Implementation & Iteration
  • Own the implementation and ongoing refinement of Karoo Health care model, including risk stratification, and condition based care protocol development.
  • Translate evidence-based cardiology guidelines into practical and scalable care protocols.
  • Test, iterate, and standardize care protocols to meet the needs of individual markets and payer arrangements.
  • Ensure the Karoo Health care model supports value-based care (full upside and downside risk) through alignment of patient, provider, and payer specific outcomes.

Clinical Operations Execution
  • Operationalize the clinical model across care teams, developing SOPs, staffing models, role definitions, and training materials.
  • Identify operational bottlenecks or clinical variation and drive continuous improvement initiatives.
  • Support market launches by ensuring clinical readiness, workflows, and staffing are in place.

Care Team Leadership
  • Provide operational support to Karoo's clinical team (RNs, LCSW, MAs, etc.)
  • Support hiring, onboarding, training, and performance management of clinical staff.
  • Foster a culture of patient-centered, data-informed, high-quality care delivery.

Value-Based Care & Performance Management
  • Partner with Analytics and Finance to define and monitor clinical KPIs tied to value-based contracts (e.g., patient engagement, clinical outcomes, utilization, and financial outcomes).
  • Use data to assess clinical effectiveness and identify opportunities to improve outcomes and reduce avoidable utilization.
  • Support shared savings performance by aligning clinical workflows with payer and provider incentives.

Provider Partnership Enablement
  • Collaborate with partner cardiology practices to ensure smooth integration of Karoo's clinical model into existing workflows.
  • Serve as a key operational liaison to practice leaders, addressing day-to-day clinical process questions and improvement opportunities.
  • Support provider onboarding, clinical training, and ongoing adoption of Karoo protocols.

Quality, Safety & Compliance
  • Ensure clinical operations meet regulatory, licensure, and quality standards across states and care settings.
  • Partner with the CMO and clinical leaders on quality oversight, incident review, and continuous quality improvement efforts.
  • Maintain up-to-date clinical policies and documentation standards.

Cross-Functional Collaboration
  • Work closely with Product to inform clinical requirements for tools, workflows, and dashboards.
  • Partner with Market Ops, and Growth teams to align clinical delivery with expansion goals.
  • Provide clinical-operations input into payer discussions and new market planning.

What We're Looking For
  • Clinical background in cardiology. NP, PA, or MD degree is considered a strong plus.
  • 5+ years operational and people management experience.
  • Experience in value-based care provider organizations. Experience with participation in full upside and downside risk arrangements is a plus.
  • Experience building or scaling clinical programs in a startup or established health system.
  • Experience in a value-based provider startup organization is a plus.
  • Strong systems thinker who can translate clinical strategy into operational reality.
  • Comfortable operating in ambiguity and iterating quickly as the organization grows.
  • Collaborative, pragmatic leader with excellent communication skills.
