Job description

Integrated Health 21 (IH21) is a leading provider of worksite wellness programs, including biometric screenings and vaccination clinics across the continental United States. Headquartered in Pittsburgh, PA with offices in Syracuse and Elmsford, NY and Brookfield, WI; IH21 partners with insurance companies, brokers and employers to customize health and wellness programs with the goal of improving health and wellness of their employees and insured.

We are seeking a passionate and dedicated Nurse Practitioner to join our team. As the nurse practitioner, you will provide clinical assessments and identify individuals with elevated disease risk. Provide educational and concierge services for individuals to reduce disease risk using primary preventative strategies.

This is a PRN position ideal for Nurse Practitioners seeking flexible hours, with an average commitment of 10โ€“20 hours per week.

PRIMARY DUTIES AND RESPONSIBILITIES:

  • Conduct individual virtual wellness evaluations using telehealth software.
  • Educate all participants on health risk status related to diabetes, heart attack, and stroke prevention in addition to referring them into programs that they may be eligible for based on employersโ€™ benefits.
  • Use EMR to record, host, and document all consultations.
  • Identify risk, provide clinical consultation and create an individualized action plan based upon each participantโ€™s results. This action plan will be specific to each patient and their individual path going forward.
  • Assess depression status/risk of each patient and refer them to depression resources, based on the assessment of that patient.
  • Identify and refer each participant to employee carrier resources, based on each patient's individual results.
  • Actively engage participants in creating an action plan for their future health.
  • Provide lab oversight โ€“ review critical results and reach out to participants with any results out of established ranges.
  • Participate in the review and analysis of client based aggregate reports.
  • Participate in making prescription drug purchases. Investigate and identify appropriate medicines.
  • Create, review and oversee policy & procedures and standing orders, as needed.
  • Perform vaccine administration and biometric screenings in outpatient clinic as needed.
  • Triage and follow up on participant clinical issues.
  • Review protocol and retraining of clinical staff following incidents.
  • Participate in employee training as indicated.
  • Assist in creating and leading educational resources for clients.

OCCUPATIONAL EXPOSURE:

Limited exposure to blood-borne pathogens while carrying out vaccination administration. Employees must wear personal protective equipment, i.e. gloves, lab coats, etc.

PHYSICAL DEMANDS:

  • Prolonged periods sitting at a desk and working on a computer
  • Occasionally lifting/carrying up to 10 1lbs.
  • Must be able to lift or physically assist participants experiencing a medical emergency to the floor safely and provide medical assistance, as directed by protocol.

QUALIFICATIONS:

  • Master of Science in Nursing required; Doctor of Nursing Practice degree preferred
  • Current WV license
  • CPR certification
  • At least one year experience practicing as a Nurse Practitioner in Primary Care or Internal Medicine setting
  • Strong computer skills (ability to type and interact via video with patient simultaneously)
  • Excellent communication and interpersonal skills to consult patients and keep them actively engaged.
  • Bilingual English and Spanish a plus
