  • Mentor & collaborate: Support NP onboarding, share best practices, and partner with clinical ops/product teams.

How Success is Measured (KPIs)
  • Visits: Target 25+ patient tele visits/day
  • Flagged Patients are addressed within 24 hours
  • Documentation: Complete all notes using AI-powered tools within 24 hours of each encounter.

Must-Have Qualifications
  • Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.
  • Eligible for Medicare participation; company bills on your behalf.
  • 2+ years NP experience with complex adult populations in home health or post-acute settings; able to practice independently.

  • Thrives in a dynamic, evolving startup environment.
  • English & Spanish required.

Nice-to-Haves

  • Mission + autonomy: Lead a high-impact program that advances proactive home-based care-own your outcomes.
  • Tools that help: AI documentation, in-app training, integrated workflows; company laptop + mobile tools.
