About the Role

Serve as the central connection between Physician Services and the home health and hospice partners patients rely on for continued care. As a Referral Coordinator, you'll manage outgoing home care and hospice referrals from initial processing through start of care, ensuring each referral is accurate, timely, and directed to an appropriate participating provider. Your coordination and follow-through will help patients transition smoothly into the services they need while keeping providers, families, facilities, and care partners informed throughout the process.

This role is ideal for an organized, detail-oriented professional who enjoys coordinating care and ensuring nothing falls through the cracks. You'll process and track referrals, verify insurance participation, monitor timely starts of care, coordinate Plan of Care documentation and provider signatures, and maintain accurate information within the patient's electronic medical record. Through proactive communication, strong follow-up, and a commitment to exceptional service, you'll help create seamless transitions between Physician Services, home health, and hospice while ensuring patients remain connected to the right care at the right time.

Hours 

  • Monday to Friday, 8 a.m. to 5 p.m.

Required Qualifications 

  • High School Diploma or GED
  • Minimum 1 year of experience in a health care setting

Preferred Qualifications 

  • Physician office, home care or hospice experience
