Description:
As SSJC expands our outgoing referral team, we are seeking a candidate who excels in positions with a high degree of public contact, is respectful and courteous with patients, staff and providers and has a strong focus on patient satisfaction. The referral coordinator serves in conjunction with all internal clinic departments and must navigate, compromise, and negotiate successful tracking and coordination of referrals between SSJC and outside clinics. The ideal candidate possesses a strong attention to detail, ensuring accurate tracking of all referrals.
Job duties include, but are not limited to the following:
  • Communicate with patients to ensure follow-through with referrals.
  • Support doctors and patients in synchronizing appointments, authorizations, and tests taking place in different clinics.
  • Coordinate the process of referring patients to appropriate clinics.
  • Update records to assure all parties have accurate information.
  • Routinely make reminder phone call reminders to referral patients in advance of appointment
  • Keep in-office provider up to date with questions, requests and concerns regarding the status of patient referrals, care coordination or follow-up status.
  • Answer, respond and document phone calls, request and questions from patients in a timely manner. Calls must be accurately managed or redirect as appropriate.
  • Routinely provide patients with other clinic and community-based resource materials as appropriate.
Education:
  • At least one year or more clinical office experience, but two or more years of experience is preferred

The employee must communicate professionally, respectfully, and effectively with patients, visitors, clinicians, coworkers, and vendors, including in busy, demanding, or stressful circumstances. The employee must maintain professional composure and consistently perform assigned duties throughout the scheduled work period; manage routine workplace stressors and feedback without disrupting patient care, patient-facing operations, or coworkers’ work; and use established de-escalation and escalation procedures when appropriate. The employee must exercise sound judgment; maintain appropriate workplace boundaries; receive and respond to routine feedback and direction; protect confidential patient and business information; and address patient or workplace concerns through established supervisory and safety procedures. These functions are essential to the position.
