We also provide nursing home support, care management, and in-home care through our Essen House Calls program. Job Summary

As a Quality Intern at Essen Healthcare, you will support efforts to improve patient care and health outcomes through data-driven initiatives aligned with HEDIS (Healthcare Effectiveness Data and Information Set) measures. This role involves clinical data abstraction, patient engagement, and participation in community-based health initiatives-all contributing to Essen's value-based care and quality improvement goals.

Responsibilities

1. Chart Review & Data Abstraction

a. Review electronic medical records to identify and close care gaps for HEDIS measures such as diabetes, hypertension, cholesterol management, and preventive screenings.

b. Conduct diagnosis review and verify completeness of documented conditions.

c. Abstract critical data points including demographics, procedure and diagnosis codes, and service dates in alignment with HEDIS guidelines.

d. Participate in team overviews and reviews of abstraction findings.

2. Patient Outreach & Education

a. Conduct outreach calls to patients with uncontrolled diabetes or hypertension, or those with pending lab orders, to encourage timely follow-up and adherence to care plans.

b. Educate patients on the importance of chronic condition management and help schedule follow-up PCP appointments or lab visits as needed.

c. Perform SDOH (Social Determinants of Health) screenings by calling patients to assess needs related to access to care, food insecurity, housing, and transportation.

3. Community Engagement & Events

a. Participate in a full calendar of patient engagement events, including Saturday health fairs and plan-sponsored community events (typically 10 AM - 2 PM).

b. Assist with at-home services such as eye exams and blood pressure checks for patients, initially shadowing a team member and eventually conducting visits independently if comfortable.

4. Medication Adherence (Part D Support)

a. Support medication adherence efforts by contacting patients, coordinating refills, confirming availability with pharmacies, and resolving pickup barriers.

b. Help improve STAR ratings by ensuring adherence to prescriptions for chronic conditions.
