This Quality Coordinator RN position offers the opportunity to make a meaningful impact on home health and hospice patients while working in the welcoming community of Martinez, California. Martinez is a historic waterfront city in the East Bay, known for its charming downtown, scenic marina, and easy access to regional parks and trails. Residents enjoy walking and biking along the Carquinez Strait, exploring nearby nature preserves, and taking advantage of convenient access to larger Bay Area destinations such as San Francisco, Oakland, and Napa Valley. Martinez provides a relaxed, neighborly atmosphere with local shops, restaurants, and community events, all within a short drive of major urban and cultural centers. In this role, the Quality Coordinator RN focuses on ensuring high-quality, safe, and patient-centered care for home health and hospice patients. You will work closely with clinical leaders, field nurses, therapists, social workers, and other interdisciplinary team members to monitor quality metrics, evaluate patient outcomes, and support continuous improvement across the service lines. Typical responsibilities include: Oversight and monitoring of key quality indicators for home health and hospice, such as rehospitalization rates, symptom management, patient experience, and timely documentation Review of patient records, OASIS assessments, and hospice documentation to ensure clinical accuracy, regulatory compliance, and alignment with best practices Collaboration with clinical managers to develop and implement performance improvement initiatives aimed at enhancing patient outcomes and safety Support for staff education related to documentation standards, infection prevention, symptom management, and regulatory requirements Participation in case reviews, audits, and root cause analyses to identify trends, system issues, and opportunities for process improvement Preparation and presentation of quality reports and data summaries for clinical leadership and interdisciplinary teams Coordination of quality activities related to surveys, accreditation, and compliance with home health and hospice regulations Partnering with field staff to reinforce best practices in pain and symptom control, end-of-life care, and safe care delivery in the home setting A typical day may involve reviewing quality dashboards and charts, analyzing trends in patient outcomes, conducting targeted chart reviews, and meeting with clinical teams to discuss findings and improvement strategies.
