Northern Manhattan Rehabilitation and Nursing Center is seeking a detail-oriented and compassionate MDS Registered Nurse (RN) to join our interdisciplinary team. In this role, you will be responsible for coordinating and completing the Minimum Data Set (MDS) assessments to ensure accurate care planning, regulatory compliance, and optimal reimbursement, while supporting high-quality, resident-centered care.

Responsibilities of MDS Coordinator (RN):
  • Coordinate and complete MDS assessments in accordance with federal and state regulations
  • Develop and update comprehensive care plans in collaboration with the interdisciplinary team

  • Monitor clinical documentation to support appropriate case mix and reimbursement

  • Requirements for MDS Coordinator (RN):
